task_destroy() implementation, fixes in as_destroy() and task_kill().
This is the first version of HelenOS that would perform complete cleanup leading from thread to destruction of address space. |

Fix programming of local APIC timer to interrupt the processor
in the proper relation to HZ. (Was hardcoded to be each 1ms,
but should have been each 10ms). |

Replace nonblocking argument of waitq_sleep_timeout with flags that specify mode of operation.
Now a flag can be used to specify interruptible sleep.
Modify waitq_interrupt_sleep() to only interrupt threads that used this flag.

Modify the hierarchy of page fault handlers to pass access mode that caused the fault.
Architectures are required to pass either PF_ACCESS_READ, PF_ACCESS_WRITE or PF_ACCESS_EXEC
to as_page_fault(), depending on the cause of the fault. |

Do not report possible deadlock on fb_lock.
This issue was causing troubles in connection
with slow framebuffer in simulated environment
when false positives triggered an infinite
recursion of attempts to report deadlock on
fb_lock using printf() and fb_putchar(). |

Complete implementation of copy_from_uspace() and copy_to_uspace()
for amd64 and ia32. Other architectures still compile and run,
but need to implement their own assembly-only memcpy(), memcpy_from_uspace(),
memcpy_to_uspace() and their failover parts. For these architectures
only dummy implementations are provided. |

Added support for IRQ notifiactions.
- seems to work correctly on ia32, amd64
- the mips kbd probably lowers interrupts when the char is read from
the port, we will have to find a way how to deal with it. |
